Clinical Evaluation of Pancreatic Pseudocyst



Abstract
Pancreatic pseudocysts are thought to develop secondary to rupture of a pancreatic duct with escape of pancreatic juice into the surrounding tissues or spaces. The fluid becomes loculated and a fibrous wall results from the secondary inflammation. There is no epithelial lining to the cystic wall. The lesser omental bursa is characteristically the site of cyst formation.
13 cases of pancreatic pseudocyst were treated at National Medical Center from 1963 to 1973.
Summary is as followings,
1. Cases consisted of 7 males and 6 females and peak age¢¥ incidence were. from. 30 to 50 years (61.59x.
2. 6 cases had history of pancreatitis, 5 cases trauma but no specific past history was obtained in 2 Cases.
3. 12 cases had abdominal mass and 10 cases upper abdominal pain. Remaining findings were anorexia & weight loss, nausea & vomitting, abdominal distention, hepatomegaly and chillness.
4. In the laboratory findings¡¯ ESR was elevated in 8 cases, leukocytosis in 5 cases and elevated serum amylase in 4 cases.
5. Radiographic examination, especially upper GI series was useful diagnostic aid.
6. Roux-en-Y cystojejunostomy was performed in 9 cases, marsupialization in 3 cases, simple
drainage in 2 cases, cystogastrostomy, cystoduodenostomy and distal pancreatectomy in one case each other. Best results were obtained in Roux-en-Y cystojejunostomy group.
